  • Approval of the preparation as a marketed product — there is none
  • The manufactured alternative’s supply

  • The pharmacist-in-charge and the state board of pharmacy
  • Refusal, where a request would require operating outside the compounding exemption
EVIDENCE YOU CREATE
  • Compounding records tying components, technique and personnel to a preparation
  • Beyond-use-date rationale and any supporting stability information
  • Environmental and personnel qualification records for sterile work
