  • Runs a sample once against the approved method and does not repeat an unwelcome result without a documented reason.
  • Reports an out-of-specification result to the supervisor before any reanalysis, and preserves the original data.
  • Separates a demonstrable laboratory error from an unexplained result, and does not invalidate a result without evidence of the former.
  • Reviews the instrument audit trail as part of the result, not as an occasional exercise, and can say what a normal one looks like.
  • Keeps analyst rights and review rights apart, and does not review their own work.

HOW IT IS DEMONSTRATED AT WORK

A reviewer would look at the chronology of an OOS: the original result preserved, the phase-one laboratory investigation documented before any retest, hypotheses tested rather than assumed, and the conclusion supported by what was found. They would also look at whether this analyst has ever recorded an OOS that went on to be confirmed — a laboratory whose OOS results are all laboratory error is describing its investigation, not its samples.

WHERE IT IS COMMONLY WEAK

The weak point is the space between an unexpected result and its explanation. Testing into compliance rarely looks like misconduct from inside: the second injection is run to check the first, the column is changed because it seemed tired, the sequence is re-run because something looked odd — each defensible alone, and together a result selected rather than measured. Audit trail review is the other gap, commonly delegated to periodic sampling and so unable to see the single deletion that matters.
