A Media Fill Fails
One vial out of a routine media fill comes back cloudy. Decide what that single unit means, what happens to the product filled since the last good simulation, where the cause is, and when the line can fill again.
What a scenario is not
A scenario is a judgment exercise, not an assessment and not a compliance verdict. Finishing it evidences that you worked through it — nothing more — and the dimensions it scores are SPEQ’s, not any regulator’s.
To check that sterile medicines are filled without letting germs in, a factory regularly fills vials with a growth broth instead of medicine and watches for anything growing. One vial grew something. You decide what that means for the medicine already made, and when the line can be trusted again.
An aseptic process simulation (APS), or media fill, imitates the routine aseptic process using nutrient media. EU GMP Annex 1 (2022) sets the target at zero growth and treats any contaminated unit as a failed APS. It then requires an investigation, corrective measures, normally at least three successful consecutive repeats, a review of batches made since the last successful APS, quarantine of product from the line, and no production until revalidation. FDA’s 2004 aseptic processing guidance states that any contaminated unit is objectionable and should be investigated.
A media fill is one of the few direct measurements of aseptic control a site has. A single positive unit is a small number with large consequences, and the line stays down until it is resolved. That is why there is pressure to explain it away.
You are the sterility assurance specialist. The site holds an EU manufacturing authorisation and also supplies the US. The six-monthly APS on vial line 2 filled 7,500 units.
At the final inspection after incubation, one unit is turbid. Each choice changes what you can still do.
The inspector has set the turbid vial aside. Production asks whether one unit in 7,500 is within the acceptance criteria.
This is a judgment exercise, not a quiz: your choices change the situation. It is scored across response · investigation · productImpact · requalification — the same dimensions an inspector effectively weighs when they follow the thread of a real deviation. There is no compliance verdict here; the debriefs explain consequences.

Finishing this exercise is recorded. What it evidences — and the things it does not — is set out against the published standard.
