How to Assess a Temperature Excursion
Decide whether product exposed to out-of-range temperature is still usable.
What a how-to is not
A how-to is SPEQ’s practitioner method, not a procedure. It does not replace your own SOP, it is not a validated approach, and the judgement calls in it belong to your quality unit.
A temperature excursion is a departure from a product’s labelled storage conditions during storage or transport. Assessing one means determining, on the basis of stability data, whether the product’s quality was affected and whether it can still be used — a Good Distribution Practice obligation that protects patients from degraded product.

Capture the excursion accurately
Capture what actually happened rather than what the alarm reported: the full time-temperature profile, its duration, the minimum and maximum reached, and the monitor’s calibration and placement. An excursion summarised as "out of range for six hours" cannot be assessed, because the product’s response depends on how far out and for how long, and those are the two facts a summary discards.

Quarantine pending assessment
Quarantine the affected stock pending assessment, physically or by system status, and make sure the hold reaches everywhere the product could be picked from. The purpose is to prevent the product being used while the question is open, and the common failure is a quarantine applied in one system while the goods remain available in another.

Assess against stability data
Assess the profile against the product’s stability data and the approved excursion allowance, cumulatively across the product’s life rather than as an isolated event. This is the step that requires actual data: an assessment against the alarm setting measures the monitor, and an assessment against intuition is not an assessment. Where the stability data does not cover the excursion seen, say so — that is a finding, not a blocker to be reasoned past.

Decide disposition
Decide the disposition on the assessment and record it with its basis: release, release with reduced shelf life where the registration permits, or reject. The decision is a quality decision and belongs to the quality unit, because the commercial cost of rejecting falls elsewhere and the pressure is predictable.

Investigate the cause and prevent recurrence
Investigate the cause and act on it, distinguishing a packaging or lane failure from a handling failure at a specific point. Most repeat excursions come from the handover points nobody owns — a dock, a customs hold, an unattended weekend — and an investigation that concludes at "transport delay" has described the event rather than located it.
