HOW IT IS DEMONSTRATED AT WORK

A verifier would look for a hazard analysis with reasoning attached, monitoring records that include out-of-limit readings and the actions taken, and a validation basis for the critical limits. The clearest evidence is a deviation handled to its product decision: caught by monitoring, acted on, the affected product dispositioned, and the plan reassessed if the cause suggested it should be.

WHERE IT IS COMMONLY WEAK

Two patterns recur. Critical limits set where the process already runs, so monitoring confirms normality and can never be exceeded — a control that cannot fail is not controlling anything. And verification collapsed into monitoring: the same person doing the same check, so nobody is asking whether the plan still describes the hazards, which changes whenever the product, supplier or line does.
