  • Treats the source record as the origin of the data and the case report form as a transcription of it, never the reverse.
  • Raises a query when data are internally inconsistent rather than resolving the inconsistency by choosing a value.
  • Recognises which changes to a study need an amendment and prior approval before they are made.
  • Reports a safety event through the study’s reporting route within its timeline, regardless of whether it appears related.
  • Keeps the trial master file contemporaneous, so the file would reconstruct the conduct of the study without anyone explaining it.

HOW IT IS DEMONSTRATED AT WORK

Evidence sits in the query log and the file. A monitor would look for queries this person raised rather than only closed, source-to-CRF verification with discrepancies recorded rather than silently aligned, and a trial master file whose documents were filed when they arose. A file assembled before an inspection tells the inspector when it was assembled.

WHERE IT IS COMMONLY WEAK

The persistent failure is the file reconstructed at the end and the protocol deviation that was never recorded because the study still worked. Both come from treating documentation as reporting rather than as the study’s own record. A subtler one: queries closed by agreement with the site, where the value changes and no source supports the new one — which is a data correction wearing the clothes of a clarification.
