What a definition is not
A definition is SPEQ’s plain-language decode of how a term is used in practice, cited to the documents that define it. It is a practitioner reference, not legal or regulatory advice, it does not replace the definition in the source, and where a regulator’s wording differs the regulator’s wording governs.
Right-first-time is a quality metric and operating philosophy measuring the proportion of work — batches, records, transactions — completed correctly without error, rework, or deviation on the first attempt. A high RFT rate indicates well-controlled, well-understood processes; a low or declining rate signals process, training, or documentation weaknesses that generate deviations, investigations, and cost of poor quality. It is a leading indicator: it degrades before the failures it predicts become recalls or inspection findings.
RFT reframes quality from "did we catch the errors?" to "did we avoid making them?" — which is why it correlates with lower cost of poor quality and with maturity. Every unit that is not right first time carries the hidden cost of rework, investigation, and delay.
As a metric it must be defined precisely (what counts as "not right," at what stage) to be comparable over time and across sites — the same definition discipline that governs all quality metrics applies.
