Aseptic Gowning Qualification Record
Qualification record for operators entering grade A and B: training completed, a visual check of every grade B gown element, microbial results at glove prints, forearms, chest and hood against the Annex 1 Table 6 limits, aseptic process simulation participation, the qualified scope and the annual reassessment date.
What a template is not
A template is a document baseline to adapt inside your own quality system. SPEQ does not approve, validate, or take responsibility for what you issue from it, and using one is not evidence of compliance.
The record that an operator may enter grade A and B areas unsupervised. EU GMP Annex 1 (2022) restricts that access to people who have passed a visual and microbial gowning assessment and taken part in a successful aseptic process simulation, and requires reassessment at least annually. This record captures each element so the qualification can be shown, not asserted.
What's Inside
How to Use It
The full section structure of this template — every section and sub-section, so you can use it as a baseline for your own site document.
EU GMP Annex 1 (2022) sets the training content (7.3), annual visual and microbial reassessment at locations such as gloved fingers, forearms, chest and hood (7.4), the APS condition for unsupervised access (7.4), disqualification on adverse monitoring trends or a failed APS (7.6), the grade B garments (7.13) and the grade B action limits for contact plates and glove prints (Table 6). The number of qualifying sessions is not set by the Annex; it is the site’s decision. The record structure is SPEQ practitioner synthesis.
