Risk is anticipated. Data and knowledge predict where quality will fail, and the system prevents it — QRM is the operating system, not a form.
What a stage is not
The stages are SPEQ’s organising device for building a risk-based quality system, not a regulator’s mandated sequence. Real programmes overlap and revisit them, and no inspection asks which stage you are in.
The organisation can say, with evidence, where failure is most likely to appear next — not from intuition but from data it already holds, connected. Process capability, environmental trends, supplier performance, deviation patterns and change history stop being separate reports and start being one picture, and the question shifts from "what went wrong" to "what is drifting, and how long do we have".

WHY ORGANISATIONS STOP HERE
Few organisations sit here across the whole quality system, and the honest characterisation is that most that claim it are predictive in one domain and managed everywhere else. The stall is data integrity and connection rather than ambition: prediction needs records that are trustworthy, comparable and joined up, and an organisation with excellent intentions and fragmented data will keep producing forecasts nobody acts on.
